› Microscopic Features:

• 2-3x larger than a mature RBC
• Low nuclear to cytoplasmic (N/C) ratio (more cytoplasm than nucleus)
• Nucleus is mature
• Nucleoli are absent
• Lack or diminished neutrophilic granules

› Normal % blood-PB, marrow-BM, lymphoid tissue-LN:

• PB: Rare to None
• BM: Rare to None
• LN: None

› May Resemble:

• Eosinophil [Compare]
• Segmented Neutrophil [Compare]
• Band Neutrophil [Compare]
• Neutrophil with toxic granules [Compare]
• Basophil [Compare]
• Monocyte [Compare]

› Differential Diagnoses:

Increased in: Myelodysplastic Syndrome Staining artifact giving rise to pseudo-hypogranularity

› Classic Immunophenotype:

• CD45+
• Lowered SCC
• CD10+/-
• CD11b+/-
• CD13+/-
• CD16+/-

• The low SCC noted in flow cytometry is due to the hypogranularity. The immunoprofile may be aberrant if it's dysplastic and associated with MDS
